The links below allow you to download the bankruptcy worksheets and debt form(s). After you have filled them out, please send them to our office. Please note that you might need to print out several pages for the debt form pages if you have numerous creditors. Once we receive the worksheets we will begin to prepare your petition for our meeting and subsequent filing with the court. Please keep in mind that this is the first step in the process. The information you provide will enable us to prepare the petition for filing, however, in order to actually file the petition with the court we will need to meet to review an sign the actual bankruptcy petition.
When gathering this information it is important to list all of your creditors and list all of your assets. Although we can pull a credit report to ensure that all known creditors are listed, it is common to have creditors who do not appear on those reports (utilities, child support, etc.). For those creditors not listed on the credit reports you will have to provide the necessary information.
